Please note that in addition to the above forms, we will also need you to complete an Emergency Card and a CACFP Form. Please also note that we must have a current Immunization Record on file for every student that attends our Academy.
Should you be enrolling your child in Kindergarten we will also need a copy of his or her birth certificate.
